At The Legacy Care At Home, we understand and respect the importance of the family unit, and that’s why we include the patient’s family in all decisions related to their transition from hospital to home. We invite family members to ask questions, make recommendations, and get to know their loved one’s caregiver.
We want family members to feel comfortable with their loved one’s caregiver. What Our Clients Say
Post-hospital care frequently asked questions
Why should you hire an in-home caregiver for post-hospital care?
Following discharge for the hospital, the first few months of recovery are considered to be a high-risk periods for seniors and aging adults. It can be difficult for older adults to adhere to instructions from medical professionals once at home – especially without professional assistance in the form of an in-home caregiver.
With personalized support from the team of caregivers at The Legacy Care At Home, the recovery process at home following discharge from a hospital can be safe and easy. Also, help from our caregivers can ensure that we minimize the risk of readmission to the hospital.
